As much as we have enjoyed caring for you, we are happy with you when you are able to leave our hospital again. You will discuss the exact time of your discharge with our doctors.
To make the transition period after your discharge as pleasant and safe as possible for you, we have developed a special discharge management programme. This means that social workers will discuss with you in advance, and then organise, what you need for your support (for example help from community care centres, nursing services or domestic help).
On your last day in our hospital you will receive a doctor's letter and, if required, a certificate for your insurers and, where applicable, a certificate of incapacity for work for your health insurance fund and your employer. As the administrative discharge takes place automatically, you do not need to take care of anything else on site. If you have used the telephone, you only need to return the corresponding card to the information desk. An invoice for your statutory co-payment will be sent to your home.
Doctor’s letter (Arztbrief)
On discharge, you will receive a provisional discharge report from your ward doctor. We ask you to present this to your GP promptly for further care. We recommend making a note of tips and advice from our therapists, nursing staff and ward doctors.
A detailed report will then be sent to your GP by post. If you will continue to receive nursing care after discharge (e.g. in a nursing home or from a community nursing service), you will also receive a nursing report.
For future stays, including at other hospitals, please always mention your stay with us.
Co-payment
In the case of an inpatient hospital stay, you are obliged under Section 39 (4) of the German Social Code, Book V (§ 39 Absatz 4 SGB V), to make a co-payment of €10 for each day or part of a day, limited to 28 days per calendar year. Hospitals are legally obliged to collect this personal contribution on behalf of your health insurance fund. For this reason, you will receive an invoice for this co-payment after your discharge. Please note that if payment is not made on time, dunning proceedings will be initiated and you must expect an enforcement order to be issued.
The co-payment does not apply in the following circumstances
- you are under 18 years of age,
- your stay exceeds the 28-day limit,
- you are exempt from co-payment,
- your stay is for an inpatient delivery.
Social services
There are situations that bring about considerable changes for the rest of your life. Our social services team will then be happy to advise you on the consequences in terms of employment law and taxation. They also organise what is known as follow-up rehabilitation (Anschlussheilbehandlung, AHB), to which you are entitled for certain illnesses.
As a rule, we will approach you about this option ourselves. However, please do not hesitate to contact our social services to arrange an appointment for a consultation.
Discharge management
Counselling for patients and relatives is an important component of discharge management, ensuring seamless continuing care after your stay in our hospital.
Discharge management is available to all patients – especially those who will need further support at home after their hospital stay.
We provide individual advice on all questions relating to further care and nursing at home, and in cooperation with you and your relatives we arrange and organise all necessary assistance and contacts. Please feel free to contact us. Further information can be found here.
