Appointment request – gynaecology

Please bring the following documents to your appointment:

• Any current findings or imaging (e.g. CDs) relating to your symptoms.
• Health insurance card from your health insurance fund
• Referral form (Überweisungsschein) from your specialist (gynaecology or urology)

Important: Unfortunately, we cannot accept referral forms from general practitioners.
